Release forms?

I am curious is to how many documentarians get a release form for people they interview. What are your thoughts on this?

Last year I recorded a beauty pageant for a friend. Nothing receiving high press or anything, just a local competition. I cut a Blu-Ray, DVD and a trailer, and the trailer was used online for promotional purposes. Anyways, one of the contestant's mother demanded that her daughter be removed from the trailer because she was shown in a swimsuit. The girl was 17, so I can understand to a point, but come on, there's 3 cameras shooting at the stage, a big sign that says video recording in progress and another sign that says video will be sold and promoted on youtube. Long story short, because she threatened legal action, I removed the girl from the trailer, and replaced her with another girl in a swim suit, and I now take release forms to any gig.

Sounds familiar, and of course a 17 year old still requires the signature of a parent / guardian for the release to be valid.
