Release Form Templates

[bermuda edge productions]

Could anyone direct me to a link to a good general commercial release form template? Would you reccomend altering a template to personalize it to better fit my situation and reinforce validity?

I am going to be airing what is shot on a community cable access channel and there is a possibility it could later be sold on dvd.

suggestions/advice/feedback very appreciated.
