We have a shoot (today) and the rental house is desperately trying to figure this thing out. they just got it on friday and we worked on it for a while but here is the problem we encountered:

once the adapter was set, and we focused the dvx on the focal plane of the Micro. Now, the prime we used (Zeiss standard speed, 11mm?) and the focus on the lens was off. A hand truck that was 3.5' away was only in focus at infinity.

Any tips on how to fix this issue?

That sounds like a flange focal distance issue. I ran into this same problem. Basically, you need to adjust the distance of the GG. You can do this with either a collett wrench or you can do what I did; whcih is remove the gg and set the washers behind it to push i out a bit. Worked like a champ for me.
