I haven't seen this book mentioned so I thought I'd throw it out there. I found it very valuable. In addition to some actual practical setups and improvisations from specific award winning films, "Masters of Light: Conversations With Contemporary Cinematographers" by Dennis Schaefer, Larry Salvato gives a wealth of conceptual advise (a full chapter each) from:

Nestor Alemendros
John Alonzo
John Bailey
Bill Butler
Michael Chapman
Bill Fraker
Conrad Hall
Laslo Kovacs
Owen Roizman
Vittorio Storaro
Mario Tosi
Haskel Wexler
Billy Williams
Gordon Willis
Vilmos Zsigmond

Enjoy,

It's not really a "how to" book. The writers interview each cinematographer extensively mostly to learn each individual's approach to utilizining camera, lens, lighting and filmstock as an artist. Yes, there's a lot of practical information such as "location lighting" but it's more about artistic concept and how each cinematographer executes to achive that concept.

Great book. I had my copy stolen when I was in school, but found it at a used bookstore. They still made me pay whatever they paid for it from the bum who stole it from me, but it was worth it. It's a great look into the minds and working habits of some great DPs.
