Where do people find/use realistic backgrounds for their key-ing work. I can't seem to find any good sites. Specifically I need a doctor's office...

Visit your local doctor/clinic/hospital and politely ask if you can take some photos? Security/privacy concerns aside, it shouldn't be too difficult, if you explain what you need and why, and let them veto the resulting photos (I'm presuming you'd use a digital camera). Taking your own photos will also let you minimise awkward lighting issues, so that you can match your live footage more easily.
As a more general answer though, I've found many photo sharing sites like PhotoBucket a great source of generic backgrounds. But keep an eye on copyright licensing issues, especially if you're working on a commercial project.

The best thing is to take the photos by yourself, but if you need a photo of a paradisiac landscape and you can't go there you may try something like flicker and use cc licensed photos.
