Have your actor wear the appliance (I've found that foam like is used in headrests and seats is really good. The resistance it puts on the blade as it pulls out looks really realistic. then put your blood into the foam or pierce a blood packet on top of it.) When you pan down if she turns slightly it will look like she's pulling it from her stomach.
