P2 Clips Duplicating as I Log

Hey, quick question: we're in the process of logging P2 footage from our current project, shot on an HVX 200 at 720 24P. As we go into the browser window, and add notes, scene #, etc., Final Cut will randomly duplicate the clips. It doesn't happen every time, and doesn't seem to happen for any particular reason. When it duplicates, it creates a new file name, putting a 1 at the end of the original file name, creating, say, 0003SV1 from the original 0003SV. Because of this, it's taking up 1 - 3 GB of storage for each additional clip. So eventually, since we're dealing with a feature, it will become a problem. Any advice?
 
I know that happens when you import/transfer the same clip more than once, (it will also add sometimes consecutive letters too), but never had it do it to me randomly. I've double clicked on the "add to queue" or "add selection to queue" by accident and had it happen, but haven't had FCP do it on its own.
