Need Release Form ASAP : Kindly Help

Hi There,

I am on a short visit to India. My Hard Drive crashed and i lost all data, and thus lost all the release forms and electronic documents etc.

I tried doing an online search but could not find much. Can somebody kindly show me a link to a sample release form for personal interviews and also if possible a location/property release form too. (you could also PM me a doc file link or email it to me as an attached doc file)
