My Wide Angle Lens Problem.

Just installed my wide angle I'm pretty sure I did it right I used a 72 to 62 adapter and it fit ok, but there are two problems

1: my autofocus doesn't seem to be working with the lens attached... images are blurry and

2: in each of the for corners I can see what I think is part of the lens it makes for black spots in each of the four corners

can someone help me thanx in advance
