Missing Media ?

I have a clip that keeps disappearing out of a FCP capture folder. I'll open the project and it asks if i want to reconnect. It has happened to the same clip twice now. I have to keep dragging it from my back up to replace. Can someone tell me why ?
 
Silly questions, but the first ones that we need.

1 Are you changing the file name?
2 Are you changing the file location?
3 Have you saved the project?
4 Did you "Re-Connect" media, or just "drag & drop" the clip back into your bin?
5 If yes, to #4, did you delete the "offline" media from the bin?

This happened to me recently

This, or something like this, happened to me recently. It did not repeat or happen again, but I had no backup. To make matters worse I had captured 15 tapes on 2 different computers and had accidentally labeled 2 clips the same. When I went to batch capture, you guess it, I used the wrong tape. I found no info on the web and finally called an old friend. He said it just happens sometimes with FCP. I have been using FCP since version 3 and it has never happened to me before or since, knock on wood. My answers to the above questions would be NO, NO, Yes, N/A, N/A.

I would love to hear if I missed something and if this has happened to others.
