Manfrotto Head Not Panning/Tilting Smoothly?

I've been using a Manfrotto 516 head for a while. While it is a decent head for a tripod, lately it has been doing this weird "clicking" and "snapping" when trying to tilt and sometimes when trying to pan. This has grown to be a very annoying occurrence because I use this head to shoot fashion shows and often times I have to do slight tilts just to accommodate for the different heights of the models. It sucks to have to do a slight tilt upwards and have the whole shot shake slightly because the tripod is doing its' weird clicking thing.

Bear in mind I've experimented with the adjustment knobs many times. When I loosen the "drag" as much as possible and have the head tilting rather freely I seem to only feel the clicking/snapping at the most extreme upwards and downward angles. As I slowly increase the drag, the angles that the clicking occurs at become increasingly close to each other which in turn causes more clicks for minor movements.

Does anyone have any suggestions on how to fix this?

Thanks for the help!
