After my son used an earlier zoom/focus/iris controller I made after viewing other posts here, he was disappointed that he could not go from manual to auto IRIS, or manual to auto focus easily. Not to disappoint, I decided to try to figure out how to make it work. I had previously used a 3.5mm 3 conductor (stereo) jack, but seeing the commercial controllers had 4 conductor jacks.

There have been a number of posts discussing zoom and iris/focus controllers, but I had not seen any that clearly identified the correct wiring scheme for adding manual/auto switching for focus and iris control. After scouring the internet for a diagram, I finally came up with one. Instead of drawing a nice diagram like others have done, I've wired the iris/focus controller up and labeled the parts to illustrate how it is done.
This controller wiring scheme works well with our Panasonic AG-HMC40

Here is the 'old' controller w/o manual/auto switches: http://mefixitmyself.com/?p=236
