Keeping Lighting Continuity

For larger shoots or feature films, or any production that lasts a few days, how do you keep lighting continuity?

There is a script supervisor/continuity person for keeping continuity of the film (actors actions, dialogue, wardrobe, etc)
But, what about lighting?

Such things as keeping notes on which lights were used, how were they used (diffusion, bounced, etc), Footcandles, F/Stop, etc
I know the continuity person has generally kept track of the f/stop, but what about the rest?

Footcandles are used to keep lighting continuity, but how to you keep a record of what it was for each shot/scene?

I usually write parts of it down myself.
Not that I don't trust the script/continuity person, but I like to keep track of t-stops myself;)
For lights I generally ask my gaffer before the shoot to write down each and every Light we used and its position etc.
Sometimes I have an intern writing that stuff down. Its great for them, because they see how every set is lit in detail.
But keep an eye on it, especially on the first few days.

I write down T Stops for every setup und more importantly the Contrast Ration of the Setups.
Thats what really comes in handy, if you have to reshoot a closeup two months later.
So that i know if the Fill was 2 or 3 stops under key etc.

The script person writes down the basic stuff like the T-stop. Nothing about the type of lights used, where they were positioned, and how, and the footcandle reading.

Is there a way to do it in a diagram way, something not complex?

The script person has a proper sheet they need to fill out. Is there anything like that for me to use for this?
