Indemnity Contract Needed - Need help

I'm getting ready to shoot a music video inside a night club. I have general liability insurance, however the club's lawyers have requested I draft up a contract of indemnity that includes the following language:

This agreement is made upon the express condition that the [club name], its owners, partners, landlords, agents and employees shall be free from all liabilities and claims for damages and/or suits for or by reason of any injury, or death to any person or property of [my name], its agents or employees, or third parties, from any cause or causes whatsoever while in or upon said premises or any part thereof during the term of this agreement or occasioned by any occupancy or use of said premises or any activity carried on by the [my name] in connection herewith, and the [my name] hereby covenants and agrees to indemnify, defend, save and hold harmless the [club name], its owners, partners, landlords, agents and employees from property damage and all liabilities, charges, expenses and costs on account of or by reason of any such injuries, deaths, liabilities, claims, suits or losses however occurring or damages growing out of the same.

Are there contract templates available online that address this indemnity language? If so, can someone point me to the right place?

Thank you!

If you agree with those provisions why not just drop it into your existing deal memo lock, stock, and barrel. You don't need a whole new contract just to add this paragraph. Of course it would be very wise to have your attorney review it with you first so you really understand what rights you might be signing away in all the legal gibberish.

I personally would not sign the indemnity in that form, which is very broad--possibly broader than the coverage of your liability insurance. But I am pretty cautious in such matters. You should at the very least have an attorney review the language. As knowledgeable as the members of this site are, you should not be looking for legal advice here.
