Image Distortion using Redrock

I'm using a DVX100 with a Redrock Adapter and I'm getting a distortion in the sides of the frame that are sort of like an opposite fisheye effect. Here's a shot of what I'm talking about. Look at the right edge of the poster's frame. The lens is a nikon 135mm, and the settings on the DVX are Z81 and MF45.

Has anybody else encountered this problem? Any suggestions? Please help!

Are you using the SD or HD achromat? I haven't seen this on my setup with the SD. But this is certainly a lens or achromat issue. Do you have other lenses to try and duplicate?
 
I am using the SD achromat. I have two other lenses that produce the same results. I'm almost positive that the issue is with the achromat.

Are you using a DVX100 (non A) and if so could you tell me what settings (meaning zoom and focus) you have your camera on when using the redrock? Also, are you using any extensions tubes on it? Really, any information you could give me about your setup would help me.
