HVX200 Snap Zoom Anamoly

I was on a shoot this week were the operators were doing some pretty flash snap zooms. My job was the 'loader' so I got to review all the footage. It seemed occasionally during a snap zoom that the camera would go out of focus, but only for one frame...

going from say 55 to 4.2 I could scrub through the snap and at one point for one frame the focus would go completely out, not a little total blur, the next frame would be clear again...I do not believe this is operator error as I spoke to the cameraman about it. Is it possible there's some sort of probably in the lens? Has anyone else encountered this? You probably wouldn't notice it until you carefully look at playback.

Thanks!

UPDATE: The following is a series of the last 3 frames of the snap zoom. The last frame is the lens at it's widest end. Please disregard any other anamolies other than the focus as those may be an issue with the photo compression. The operator told me he was on all manual, wide open, and running a 1/250 or 1/500 shutter. What would cause the lens to lose focus for exactly one frame right near the end of the snap zoom?
