Ryan explains it above.
But what I can deduce from looking at the diagram, is that the bare essentials from the setup are the top 90* tilt plate, and the strong fluid head. As a result, you must pan the fluid head to roll the whole rig. This is because it is mounted from a completely different angle. Usually a fluid head would be mounted on the bottom of the camera; however, this setup brings the mounting point around the back of the camera and flips it horizontally. From the looks of it, the rig would look pretty precarious, but with strong tilt plates, and suitable support holding the entire thing up you should be fine. Be mindful that your center of gravity has shifted far away from the center of your average support. So you must take proper precautions that the surface where your are mounting is suitable.
Ryan used a Panther Dolly jib arm which would be a great place to mount the setup, but as he says, any rigid surface would do.
A remote head isn't neccessary because the top fluid head will be what you use to roll the camera, essentially, by panning the fluid head.

The 90* bracket/tiltplate can go on a geared head, screwed into some wood or other rigid surface suitable for a base. Does not have to be a head, although it allowed me to pan and tilt the rig for positioning purposes.
