How is this visual style achieved?

I want to replicate the visual look of the video at the link below, not the animation or content of the video, just the visual style.

1. How is the color style achieved? Can it be done only in Premiere or do you need 3rd party programs?
2. How are the noise, and flashing effects created?
3. At around 2 minutes there seems to be some artifacts that stay on the screen. Is this just some semi-transparent graphic created in another program that is placed on another layer?
