Many surgeons wear plastic shoes/clogs so blood can be washed off. Spills are constantly occurring in hospitals, and mopping machines go by daily leaving wet tracks. The "wet floor" signs are everywhere. You can have your doctor reading an article or whatever, walk by a "wet floor" sign wearing plastic clogs, perhaps near a stairway, and you have a credible devastating fall, especially if you show the surgeon smacking the back of his head on the tile. Falls happen frequently. If you don't go the car route (with junkyard closeups of damage), the fall still works with credibility.
