Flashbacks and Flashfowards

Does anyone know the correct format for a flashback and flashfoward?

INT. SKY'S HEAD - NIGHT (FLASHBACK)

or

FLASHBACK:

but make sure you mention that it ENDS, like:

END FLASHBACK

otherwise people will continue to read-on and still think you're in the FLASHBACK.
