Feedback Questions to Ask Screening Audience

I am having a pre-premiere of my documentary with some of my close friends/people who have been following it's progress. The purpose is to get feedback on any changes I need to make/additions I need to add.

So, what are some pertinent questions I should put on a feedback form? I'll be handing it out BEFORE the video starts. This is so they can write down their thoughts as the video rolls.

My experience doing this is, you'll get a somewhat biased set of notes from friends .. than from non friends. Also they seem to want to watch to not miss anything .. then make notes.

FWIW I'd screen it first then hand out the forms so as not pre-emept anything and the bigger the screen and louder the audio the better with no booze till after you gather the forms :)

Sorry I haven't offered any questions, might be helpful here if you posted the concept, title and duration. Maybe offer a local club a 'special' screening to gather some notes?
HTH cheers.
