We are filming a large scene in a club this weekend.. I was wondering If for the extras(everyone in the club) that will be shown.. should I do ...

Have everyone fill out a form as they come in the bar...

or
have a general release form.. that I can just have everyone read and then sign below kinda like a petition....

any thoughts?

The general release should be fine. You could probably even post a couple signs outside that say that by entering they are giving you permission to tape them. I've used this before and it does work. I've had some people turn away because they didn't agree to the terms.
