Pm me with your email and I can send you a disc template and also a DVD case art template in .psd . Just give me several days. Maybe someone else can get you one quicker.
I started out using photo paper in excel. Then Photoshop and now Illustrator using "Epson Presentation Paper Matte" that I buy from B&H. I found that 8.5 x 11 was just short of fitting my single cases not to mention the bigger 2 & 3 DVDs per case that were significantly thicker. This paper is legal size (8.5x14) so you have plenty of room for standard or thicker cases. The paper looks and feels like standard copy paper but the colors and images are closer to photo paper quality.
I print on an Epson Artisan 835 which also has the tray for inkjet printable DVDs. It has and continues to serve me well, but I am at the point were larger scale jobs are going to the likes of discmakers. For the inserts I don't even use the best settings. I could see no difference between Best Photo or Text & image so I opt for the lower setting to save ink.
If you still need a template PM me your email address. My Illustrator (CS5.5) files should open the same in Photoshop.
I bought those Kirkland Photo Paper from Costco. Great price in great quality.
The template I have is for printing on Letter size paper with Epson borderless printing. I moved the printout to top left of the paper, so I only need to trim the bottom and right edge of the page.
