dumbest thing you've done on a shoot?

Years ago I was PA on a music video in San Luis Obispo and I was driving a cube van. One of the camera assistants was riding with me and asked if I could drop him off in front of the hotel. Pretty much the entire film crew was standing under the awning in front of the hotel when I pulled up. I totally forgot that I was driving a cube van that was a good 2 feet taller than the awning where you drive under and park to check in. You guessed it, there was a very large crashing sound as I peeled the top of the cube van back like a very large can opener. I don't know the total damage to the van but the hotel said that it was going to cost $15,000 to fix the awning. Damn I felt like a total moron!

Gerry and I break away from the convoy to get a shot of all twenty vehicles passing into Zion National Park. Stopped at a quite magnificent and well lit curve, we decide to swap to our shorter zoom on the 575WSL for wider coverage of the amazingly lit magic hour scenery.

In the mad rush to get the lens swapped, we inadvertantly turn off zebra (located behind a protective plastic door of course) and then fat finger the iris into manual. Naturally, the iris slid almost all the way open.

With plenty of time to hook up the 8041q monitor in the back of my truck to check color and exposure, we instead stopped our set-up to chat-up a couple granola-hot hippie chicks who stopped to admire our large mattebox.

Throughout that 30 second shot, with all the trucks rolling perfectly through the splendor that is Zion, I kept thinking to myself: "Now where the **** did my detail go?!"

We felt like real a-holes for missing that shot. Complete a-holes.

I was shooting some mountain biking that was taking place on a very steep hillside. It was mid summer, so it was really dry and dusty. After I had gotten the angle that I wanted, I packed up my gear, put my tripod in my backpack, camera in my soft Kata case. As I was walking downhill to get another shot, I slipped on the loose dust, fell straight back onto my tripod which smashed into my back and damaged me more than the tripod, and, my case with the camera inside falls to the ground COMPLETELY coated in thick dust. Luckily the camera inside was ok, and after a throrough vaccum cleaning/wet rag wiping session, the case was clean too. My back still hurt though because of falling onto the tripod....

So far....got so caught up by how great the DP got the shot to look on the monitor that when tape rolled, I completely forgot to write numbers for the shot list, even though they were being called....out loud, across the room.....for the first 3 or 4 takes. It was only when the DP asked, "Is anyone writing this down somewhere?" did I remember that I was supposed to be keeping the shotlist!!!!
