Does anyone keep camera reports?

I was just wondering how many people here use camera reports on their shoots? It's a little hard for run and gun, but for scripted stuff, how many people use them? What info do you find most useful/important? Does anyone have a camera report template for excel, or word?

I am currently working on my feature length film, and i made a little card holder to go on the back of my slate.

It also helps if you have a dedicated person to this job. He does our slate, and the does our camera reports. Now I don't know in fact if what I am doing is actually a camera report, but i log each take and all that good stuff.

By the way I'm 16.

Script girl always keeps logs of time code; scene/ take; if it was a good or bad take; misc notes, etc.

By the way, I'm 21.

For DVX shoots I don't usually keep reports or logs. There is no lab involved and we don't usually use a double system. But on a HD feature I worked on I kept reports and logs like it was film for continuity and because... well the first wanted me to. If you are working with a lot of filters, lenses, etc. it helps to keep note of what scenes had them etc.

For film obviously you need to keep reports. Go to your local lab and pick some up for free. Or grab a copy of the Camera Assistant Handbook.

If you are shooting a lot of footage - for example for a feature - you need to keep logs of every shot. It will help your editor sort through it - even if you are the editor.

We are halfway done with our feature and have over 20 hours of footage at 720P - i cant imagine sorting through that - but our scripty has logged all time codes - good and bad takes, bloopers - etc. So when we review the footage we know what to look at
