Does anyone have a music release form template that I could have?

I couldn't find any conclusive results from a search, but I was wondering if anyone had a music release form, say a .doc file that I could have, and fill out with my own production info to then send to the artists I am using the music from?
 
I don't think a cue sheet solves his problem, and I know it does not solve mine. I have an original piece that I need to get rights to for one year of unlimited use (in conjunction with a specific project/client).

On that sight there is everything you need - all kinds of releases and templates. If you still can't find it, shoot me an email and I'll email one to you when I get back on sunday.
