Documentary proposal samples?

Do documentary proposals need to have a title page, appendixes and a table of contents? I can't imagine funders will just receive a piece of paper of your proposed ideas.
 
Sure, I think it would be totally appropriate to start your proposal with a question if it made sense for your documentary idea. In fact, I think it's a great technique to build intrigue and capture the attention of the reader right away!
