Crew Release Form

I am looking for a crew release form. I am having a hard time locating one. The talent release forms are easy to find.

You might have to google to find those forms or make your own based on what you find on the net. There is a CD-ROM I bought a few years ago called MOVIE FORMS PRO that has over 60 production forms on it but nothing about releases for the crew.

why would you need a release for a crew member? are they going to be on camera? if so, you can use a talent release. If they are not on cam, there is no need for a release.

Now, if you need them to keep their mouths shut about what you are shooting, then they have to sign an NDA (non-disclosure agreement) and they are easy to find.

If you are looking for a form that would specify their role on the project and their pay, then what you need is called a 'Deal Memo'

ND, maybe you should explain exactly what you're looking for. By the sounds of it to me, you're looking for a form to imdenify you of any liabilty for a crew member that injures him/herself on set. Is that right?

Wow, this was exactly what I was searching for! I couldn't find anything on it, lots of other release forms, but none for the crew.

Yes, I'm looking for one that protects me from liability if someone decides to burn off their finger on a light, or stab themselves with a prop knife, or more importaintly decides to goof around and break something that belongs to the location (like a broken window or dent in a wall).

does anyone have an example of something they've used, or do people not use them?

Thanks,

JF

sadly such a form is not. you as the producer are responcible for everything that everyone does on your set.

heres the deal, get one good gaffer. no one touched hot lights, not one plugs or unplugs a damned thing but him. get a trusted prop manager, just a friend who isnt a moron. no one but you him and the actors who need to touched a prop.

and so on. keep a tight ship, and you should be fine.

also get location insurance, it isn't to high, well depending on the location, and this can cover the location and the people on it. it will not cover lost or broken gear.

Location insurance won't cover broken gear, but equipment insurance will. That's why you have to show proof of insurance (or some sort of surity bond) when you go to a equipment rental company. You should be able to purchase that as part of your insurance package.
