Credits

I'm putting together my credits for a film I've recently made. I'm just wondering if anyone knows the standard practice for crediting people in a couple of situations:

1. the location sound was recorded by two people - each did an equal number of days on the project. One of the recordists work makes up 95% of the final film (not because he was a better sound recordist, but because much of the film was reshot, and also a bit of a coincidence). So, are they both credited as sound recordist equally? Also, just out of curiosity, let's say the recordist who only recorded 5% of the film's dialogue actually had none of his work included in the film - would he still get a credit?

2. The DP did everyday except one, does this mean when I credit the DP both of their names will have to be there?

In editing credits you will sometimes see "addition editing" perhaps a similar approach can be used here?

I just produced my first feature film and we had some similar issues with credits. If the sound recordists worked the same amount of time, I would credit them equally regardless of how much was used in editing/actual film.

For the DP credit, I would credit your main DP and maybe give the other guy "camera operator" credit? I'm not sure if that's the case, just guessing.
