Changes to characters physique?

Looking for different ways to possibly pull off long term changes to a character. Here's the scenario- Going from a very physically fit person, the character is in an accident, leaving him in a vegetative state for a long period. In that time, a person would lose a large amount of weight and muscle mass. He would be gaunt and boney. Make up will only go so far, so could a body double be used believably?

have you though about head replacement in post, use your body double well for the body :) and then in a shoot the head (your actor) for the head,

The hard part will be to get both actors to move near enough identically.

I thought about it, but a 200+ person's head on a stick figure's body would really stand out. I know some actors have gained/dropped a lot for a role, but I think this is too drastic for low/no pay. If I were a big studio with a big studio budget, I'd tell them to shut up and sweat harder, they need to lose another 60 lbs...

What about getting a super skinny actor, unhealthy looking actor and making them up a bit fatter and more healthy looking for the start of the story and then no makeup later?

I suppose that might have some possibility, but make up alone wouldn't cut it. They would need to bulk up at another point.
