If you are shooting 35mm, the lab you are using will give you free report books. Before a shoot one of the stops is to the lab for report books, extra cans, and bags.
well i might be cutting it close so i just need some camera reports and i found im shooting 16mm not 35mm. so if anyone has like a pdf or or image file it would be appericiated. i shoot in 2 days.
Okay, either way...lab reports are very specific and not the typical shot list that you see used for digital productions. They vary from lab to lab. But essentially you need scene number, take, fstop, dial, footage, general remarks.
Along with a framing reference, and timing instructions to the lab including transfer speed, telecine type for your dailie, emulsion number, negative type. Shooting 16mm, you are going to need to know what stock you are using, and also have extra film cans, cores, and bags. Else you won't be able to cut the reel in the event you have left over film.
So again, call your lab and tell them to overnight that stuff.
