How or what do you use to plug your sound feed into to monitor while booming?

Several options. A duplex cable, a headphone amp drawn off the incoming line, or a wireless feed from the mixer.
If you are talking a two man ENG type situation then the boom op will also have the mixer on his chest and then just listen off that (there is a switch to monitor the signal coming back from the camera).

Personally, when I'm mixing a narrative (short or feature) I use wireless for my boom op to monitor. I already have my comteks (well, ListenTeks actually) out on the scripty and director, so I just put a third on the boomer and away we go. One less wire going back and forth, and one less kink in the chain if something goes wrong.
