Beachtek DXA SLR- Loud Tone in Both Right and Left Channels

I just did my first recording test with my new Beachtek DXA SLR. I am using it with my Canon T2i and an Audio Technica Shotgun mic AT835b plugged into the right channel XLR. I have all my specs set to Initial Setup instructions as listed in the operating manual for the DXA SLR and I am set to Mono. Any idea what I am doing wrong?

1) We don't know what YOU mean by "loud tone". It will be difficult to help you without hearing an example.
2) WHERE are you hearing this "loud tone"? Are you hearing it in the headphone monitor output from the DXA? Are you hearing it (1) from the DXA itself, or (2) through the DSLR, or (3) played back from the DSLR?

What happens if you are just using your mic, the Beachtek box, and your headphones. Disconnect the camera and put it in the next room. If you just listen to the microphone into the DXA through the headphones, do you hear this sound?
