Anyone have samples of a Film/Video Production invoice?

I have done invoices without a problem till recently a big client wants the breakdown on equipment, manpower and so forth on a video production for DVD for behind the scenes on a popular reality show. So I would need day rate, Rental rate and everything else. So if someone can show me an example of what I need done, I would greatly appreciate it. Thank you
