Anyone have a (link to) a good release form?

[morten]

yeah yeah search I know but if you search for release forms in the forum you get about 658,352,573,925 results without actual release forms in them.

so: does anyone have a good release form or a link to one? I am shooting a huge variety of stuff and I need the form just in case.

these are great but I need something for general ENG shooting purposes - interviews with kids and random people etc.
