Anybody have contracts for Original Music being used in your features?

i have some original music in my film created by a sound person and i wanted to know if there's any general contract templates or anything out there for an agreement between him and myself in the event my film is sold?

how should pay work for this portion of the deal? does he get a percentage or flat rate?
 
I pay flat rate. Depending on the deal, he can still own sounstrack rights should you release a soundtrack only - of if broadcast, he can become part of ASCAP and work it from there.
