All my H.264 exports out of Premiere have audio sync problems...

I always export my clips from Premiere CS6 as an mp4 file using the h.264 compression. And everytime, there are slight audio sync problems when I play them back with Windows Media Player and YouTube. This is not only on my computer, last time the client even noticed it ...
VLC and Vimeo however playback fine without sync issues.

Why is this happening?

It's not about my computer and I'm sure the audio is in sync in Premiere before I export. Hence why it plays back fine with VLC and Vimeo.

Anyone else noticed this? Is it a 'bug' of Premiere? What can be done about it?
Many thanks. This is driving me nuts.

Yes I noticed this on my last 2 projects using CS6.
When I export to .m2v (for DVD) it's fine, but .mp4 has a slight sync problem when I play back from windows media player.

I dont have the problem on my CS5 machine.
