Does anyone know where I can download a good actor's release form?

You could modify this: http://www.48hourfilm.com/release-form.pdf

The Minnesota Film Board has a nice selection of release forms on its site:

You are welcome to use mine. It's one I got from Lloyd Kaufman (somewhere in his "Make Your Own Damn Movie" book he says to email him if you need contracts, I did and he did) edited for my use.
