501 Head is Loose

When I first bought my Manfrotto 501 hdv head it was smooth, but three years later and it has an annoying amount of play on the x/z axis. I've tried tightening the bar underneath the head, but it doesn't seem to have any effect. Any help would be greatly appreciated!

there seem to be screws underneath the 501 that can be easily loosen off. One fell off and the other 2 start to be very loosen and then My 501 start to get stuck on my slider. I need to get those replace to see if it's going to have any improvement.
