1 out of 6 offload problem

I did some offloads today to P2 store. When I imported the files via Mac P2 import
I missed 2 cards, where I'm 100% sure off a succesful offlaod to the P2 store before.
Does any of you recognize or experiences the same?
 
damn Barry, you save my day because I was just planning a reshoot.
Can we make this a sticky because this IS important. Indeed i just got my new mac pro and this was my first import with the new one but never pay attention to that.
 
Has anyone seen a problem with the P2 cards not displaying the ICON properly? I had a client who offloaded the media to an external drive and verified each clip by clicking on the ICON and checking each take. It seemed to work OK until the very last card of 8 which did not display. He transferred anyway, and has not been able to open these files in FCP. I work in Avid and still had not erased the card...I brought the media into Avid via the HVX and it shows up fine, although within the camera the ICONs do not display -they're grey with an X through them. Very strange behaviour and I'm checking to see if anyone else has had this issue. I ran a search and could not come up with this specific issue.
