Roundtrip DNG from Resolve > Premiere. Some seq's link properly, some are incorrect.

So, this week I've dedicated myself to nailing down this workflow for a roundtrip Resolve > Premiere > Resolve. I'm so close to having it, but I'm experiencing a strange phenomenon and I can't figure out what's happening.

Here is my process:

1. Import DNG sequences into my Media Pool (in Resolve)
2. Export QT proxies from the Deliver tab, unchecking "Render each clip with a unique filename" so that we can relink later to matching filenames.
3. Import QT files into Premiere, do an edit.
4. Export XML from Premiere.
5. Import XML to Resolve from Conform tab, unchecking the "Automatically import..." box.

Once I get to step 5, I'm able to import the sequence and I can see my cuts. Some of the clips that have been linked are correct, but many of them are wrong.

To rule out the XML document being incorrect, I've imported the XML into FCP as a test and it operated perfectly. I've also tested EDL's and AAF's, and they all have the exact same outcome. I've tried to muck with the conform settings in the Master Project Settings but I'm coming up short. I'm rather certain the issue lies in the ingestion on Resolve's side.

I've been following this tutorial to a T.

Anybody have a similar problem? Any suggestions?

Hardware: Mid 2010 Mac Pro, 32 GB Memory, 2x2.4GHz Quad-Core.
Software: OSX 10.8.3, Davinci Resolve 9 Lite (9.1.3lite.019), Premiere Pro CS6
 
I had a similar problem that I traced back to different source clips that had the same name, confusing the xml process. This happened as a result of my giving a clip name manually in my HPX170, so all the clips from that camera were identifiable but then they all had the same name.....doh! I manually fixed the clips usin the editing tools in resolve which was really tedious. Since then I noticed that resolve has info on fixing conforming problems in the manual. I no longer name clips that way.
