How to properly credit this person?

I wrote a script, all the dialogues and such are my work. But there is a poem within the script that's going to be written by someone else. I'm just wondering how this other person should be credited?

That would be fine, except that the title of the poem coincide with the title of the film. And even more confusing, the poem will be used as both dialogue in the film AND as lyric to a song.

If it is original dialog that it used then they must get some type of writing credit:

  • Second listing on screenplay
  • Story by...
  • Based on a poem by...

I agree that they need some kind of writing credit, but both the "Story by..." and "Based on a poem by..." are misleading. The first one implies that the entire story concept was by that one person, and the 2nd suggests that the film tells the story of the poem, which are not entirely true. The poem is an important part of the plot and the characters do recite it out loud but in itself it doesn't tell a story.
