The undersigned ("Company") agrees to provide the following product(s) and/or service(s) to ___________________________ for use in the motion picture now
entitled _______________________________(the "Picture")
The Company grants to you, your successors, licensees and assigns, the non-exclusive right, but not the obligation to use and include all or part of the trademark(s), logo(s) and/or identifiable characters (the 'Mark(s)') associated with the above listed product(s) and/or service(s) in the Picture, without limitation as to time or number of runs, for reproduction, exhibition and exploitation, throughout the world, in any and all manner, methods and media, whether now known or hereafter known or devised, and in the advertising, publicizing, promotion, trailers, and exploitation thereof.
The Company warrants and represents that it is the owneer of the product(s) or direct provider of the service(s) as listed above or a representative of such and has the right to enter this agreement and grant the rights granted to ________________________ hereunder.
In full consideration of the Company providing the product(s) and/or service(s) to ____________________ agrees to accord the Company screen credit in the end titles of the positive prints of the Picture in the following form "___________________________ furnished by ______________________."
The Company understands that any broadcast identification of its products, trademarks, trade names or the like which ___________________ may furnish, shall in no event, be beyond that which is reasonably related to the program content.
As it applies to any and all television broadcasts of the Picture, the Company is aware that it is a federal offense to give or agree to give anything of value to promote any product, service or venture on the air. The Company affirms that it did not give or agree to give anything of value, except for the product(s) and/or service(s_ to any member of the production staff, anyone associated in any manner with the Picture or any representative of ________________________ for mentioning or displaying the name of the Company or any of its products, trademarks, trade names, or the like.
I represent that I am an officer of the Company and am empowered to execute this form on behalf of the Company.
I further represent that neither I nor the Company which I represent will directly or indirectly publicize or otherwise exploit the use, exhibition or demonstration of the above product(s) and/or service(s) in the Picture for advertising, merchandising or promotional purposes without the express written consent.

I authorize ____________________________ ("The Film Company") to photograph me
(or the minor listed below). I have the authority to and do consent to the use of such
photographs or video (as well as the image or likeness) of me (or of the minor) by the
Film Company, to be broadcasted and distributed in public arenas, such as festivals, television and the like.
I also authorize the "Film Company" to use such photographs and video in connection with the marketing of media programs and for any other lawful use in conjunction with the "Film Company" activities.
I, (or the minor and the undersigned parent/legal guardian) understand and agree
that I, (nor the minor), will receive no form of payment or royalty as a result of the use or broadcasting and distribution by the "Film Company", (or their designee) of any photographs or video.
This Authorization and Consent Form has been voluntarily agreed to by myself or the minor and the undersigned authorized parent or legal guardian.

This is a great idea and congrats to ece for the initiative.
A few words of caution - be very careful of the "General Release" form. Reasons include: (1) no consideration is referenced in return for the granting of the release (consideration must flow both ways in order to create a valid, enforceable "contract"); and (2) the form is unclear with regard to the use of the Releasor's voice - arguably it only relates to the Releasor's image. I would also have liked to see the Release relate to a particular shoot or project - otherwise is it intended that the "Film Company" can follow the Releasor around for the next 5 years and use the images?
Short, plain language agreements are great, but sometimes what you gain in brevity you lose in enforceability.
