Without taking into account experience of the operator you go with, I'll list my guesses based on the weight range of your camera rig.

    0-10 lbs: $700
    10-24 lbs: $1000
    24-35 lbs: $1500
    35-50 lbs: $2200

    Wireless follow focus: $100-$200
    Wireless video feed: $100-$200

Steadicam. Figures are low by at least a few hundred dollars for both operator and each of the add ons and that is at a minimum, if they are going to stay in business.

Being as Patryk is in NYC, rates there are consistently some of the highest in the countrys (as befits the cost of living), but of course these days there are many operators hungry for work who are less experienced and with lesser gear to be had. Indeed it would help to know, as Eric indicated, what camera/lens package is involved and whether the wireless options are included (most working operators own and include decent lens controls but HD transmitters tend to be additional).

That all said:PSA rates are often lower by design. Last one I did was at 1/10 of my commercial quote.
