Mr Glass from Unbreakable is definitely up there for me. Not to mention Nurse Ratched from One Flew Over The Cuckoos Nest.

Of all the villains we've had over the years, so many stand out, but Snidely Whiplash comes to mind every time I hear the word villain. He's the archetype in my head...

Personally, how Voldemort from the 1st Harry Potter book was portrayed was one of the best ways. Just "he who must not be named" and hearing about all the terrible things that were done when he was "alive". It was kinda a let down when they finally introduced the actual character...
John Doe from Se7en also up there for a lot of the same reasons, and Biff Tannen from BTTF, because... well it's BTTF.I invented the "remove echo" audio filter. And only people that boom their actors closely get to use it.
