View Full Version : Does any one have a template for interview agreement form ?

I would just like to find a form to cover myself after having interviewed someone so that i can use the footage as i please broadcast/internet with their consent.
Would anyone have a template for this or know where i could find one?

I agree to the inclusion of my contribution in this video, the nature of which has been explained to me. I understand that my contribution will be edited and there is no guarantee that my contribution will appear in the final film. I agree that my contribution may be used to publicise the documentary.

I have agreed to accept payment of……. for the use of my contribution. This fee is payable on completion.

I understand that this video (or part of it) may be distributed in any medium in any part of the world.

My contribution has, to the best of my knowledge, been truthful and honest. I have not deliberately sought to conceal any relevant facts from the makers of this film.

^^^ That would do it. Maybe add that the images/video may be distributed in perpetuity.

Find yourself a standard appearance release and then tweak it. Too simple and it won't hold up. If this is an important project, have an attorney give it a once over in the state you will use it.
