How was the camera set up for this bokeh effect here:

Several images down the page you see it.

Could the same effect be achieve in full daylight using the ND filters at max?

I know it is not an f1.8 lens but when I go wide open I rarely see any bokeh

All the same rules apply -- for shallow depth of field, open the iris as much as possible, get the camera as close to the subject as possible, zoom in the lens as much as possible, and separate the subject from the background as much as possible.

That particular shot (the one with the candles) is shallow because it's a small object that is being focused on at a short distance.

Several folks have mentioned that putting the camera in macro mode helps generate more bokeh.

I have also found that macro mode does slightly help.
