It's been suggested to me that it makes sense to form an LLC.
Does anyone have any suggestions for how to do that? For example is there a web site or book I should look at?

If you don't think you quite understand it, then you probably need to consult someone. LLCs are strange beasts and you have several ways you can go with it. You might end up with something other than what you really want.

I formed one several months ago - DIY in Wyoming. Chose WY because of favorable tax considerations - NV is also good, and of course, Delaware has favorable corporate law. Lots of web sites around will help - the going rate seemed to be $400 - $800. What I did was hire a lawyer for 1/2 hour and asked all kinds of questions, showed him my paperwork, asked 'What else to I need to know?' Cost me $75 for the 1/2 hour, but beat the hell out of the $500 they wanted to do everything.
Main thing the LLC gives you is liability protection in case you get sued.

If you have partners LLC is about the best for a small corp. If you don't have partners, you might want to see about an S-Corp. I have both and the S-Corp just filters profits to my 1040 and saves some yearly IRS fees.
Best to talk to your CPA and get some advice if possible.
