So, what is the system?

Always connect with the camera off; wait 5 seconds or so, turn it on.

Ejecting: eject the card (in Mac) or the device (in Windows) wait five seconds and turn it off?

Is that how it goes?

USB doesn't present the issues that FW does.

Your procedure sounds good for either connection.
